ABSTRACT

Objective@#To understand the characteristics of different pulmonary function grades and comprehensive assessment groups among patients with chronic obstructive pulmonary disease (COPD) from community survey, so as to provide the evidence for the comprehensive community management of COPD. @*Methods@#The survey data of five monitoring sites in Zhejiang Province during 2014-2015 in the COPD surveillance of Chinese residents aged 40 years and above was collected. The patients with COPD were classified by pulmonary function according to the percentage of forced expiratory volume in the first second (FEV1) in the predicted value, and by comprehensive assessment according to modified British Medical Research Council (mMRC) score, COPD Assessment Test (CAT) score and acute exacerbation in the past year. The characteristics of different pulmonary function grading and comprehensive assessment groups were compared. @*Results@#A total of 355 patients with COPD were found, with an average age of (63.00±9.63) years. The male-to-female ratio was 3.18:1. There were 243 patients with an education level of primary school or below, accounting for 68.45%. The awareness rate of COPD related knowledge was only 1.69%. There were 257 (72.39%) patients with mild, 86 (24.23%) patients with moderate, and 12 (3.38%) patients with severe pulmonary function. The proportions of patients with childhood history of severe respiratory infection, self-reported respiratory system diseases, chronic cough, chronic sputum, acute exacerbation in the past year, CAT score ≥10 points, and inhaled medication were lower in the patients with mild pulmonary function than with moderate and severe pulmonary function; the body mass index and waist circumference were higher in the patients with mild pulmonary function than in the patients with moderate and severe pulmonary function (all P<0.05). The comprehensive assessment resulted in 158 (44.51%) patients of Group A, 134 (37.75%) patients of Group B, 1 (0.28%) patient of Group C, and 7 (1.97%) patients of Group D. The average age and systolic pressure were lower in Group A than in Group B, C and D; the proportions of patients with self-reported respiratory system diseases, family history of respiratory diseases, chronic cough, chronic sputum, dyspnea, acute exacerbation in the past year and CAT score ≥10 points were lower in Group A than in Group B, C and D (all P<0.05). @*Conclusion@#The community patients with COPD are mainly classified to mild pulmonary function and Group A in the comprehensive assessment; thus they need different levels of management.

Objective@#To investigate the impact of comprehensive geriatric assessment(CGA)on treatment outcomes of chronic heart failure(CHF)complicated with emotional disorders in the elderly.@*Methods@#A total of 216 CHF patients with emotional disorders at Henan Provincial People’s Hospital were recruited from September 2017 to March 2019 and were randomly divided into a CGA group and a control group with 108 cases in each group.The control group was given standard drug treatment and psychological counseling, whereas individualized treatment was given to participants in the CGA group in compliance with CGA guidelines.The clinical effects after intervention for 8 weeks in the two groups were examined, using measures such as Hamilton Depression Rating Scale for Depression(HAMD)-24, Hamilton Anxiety Scale(HAMA)-14, amino-terminal pro-brain natriuretic peptide(NT-proBNP), the 6 minute walk test(6MWT)and left ventricular ejection fraction(LVEF). The changes of cognitive status, nutritional status, fall risk and other indicators in patients were comprehensively assessed and statistically analyzed.@*Results@#Compared with pre-treatment data, 8 weeks of treatment for both the control group and the CGA group resulted in decreased HAMD-24 scores(Control group: 31.78±9.08, 23.69±10.16; CGA group: 32.09±8.98, 15.35±7.91; P<0.05), HAMA-14 scores(Control group: 22.38±7.09, 15.28±6.54; CGA group: 21.99±8.12, 10.48±6.82; P<0.05)and NT-proBNP levels[Control group: (4672±392)ng/L, (3279±282)ng/L; CGA group: (4861±378)ng/L, (2387±215)ng/L; P<0.05], and increased LVEF(%)[Control group(34.5±6.2)%, (39.8±7.5)%; CGA group(35.1±3.9)%, (42.5±6.1)%; P<0.05]and 6MWT scores[Control group(298±79±7.5)m, (358±102)m; CGA group(305±98)m, (402±178)m; P<0.05], with more marked changes in all the measures in the CGA group than in the control group(P<0.05).@*Conclusions@#The early application of CGA can improve the condition and prognosis in elderly heart failure patients complicated with emotional disorders.

ABSTRACT

Objective@#Select the appropriate disease assessment indicators, formulate the comprehensive evaluation group of pneumoconiosis patients, and explore the role of the comprehensive evaluation grouping in the clinical evaluation of pneumoconiosis, and provide the basis for the prognosis of pneumoconiosis.@*Methods@#Combined with clinical symptoms, pulmonary function, pneumoconiosis stage, acute exacerbation and complications, a comprehensive assessment of pneumoconiosis patients was established.138 newly diagnosed pneumoconiosis patients were divided into low risk group, middle risk group and (very) high risk group. The patients were followed up by telephone to record their health status and quality of life within one year after discharge from hospital. Analysis of the relationship between the comprehensive assessment group of patients with pneumoconiosis and symptom score, pulmonary function, pneumoconiosis stage, acute exacerbation and complications. The relationship between the comprehensive assessment group of pneumoconiosis patients and the risk events (the number of visits, hospitalization, mechanical ventilation, death cases in one year) and CAT score were analyzed.@*Results@#There were significant differences in clinical symptoms, pulmonary function injury, pneumoconiosis stage, acute exacerbation and complications among patients in low risk group, middle risk group and (very) high risk group (P<0.01) . With the increase of comprehensive assessment score, CAT score increased, the risk events increased, the difference was statistically significant (P<0.01) . Spearman correlation analysis showed that the comprehensive assessment group was significantly correlated with the number of visits, hospitalization, mechanical ventilation, deaths and CAT score in one year.@*Conclusion@#Combined with clinical symptom assessment, pulmonary function assessment, chest imaging assessment, acute exacerbation assessment, and complication assessment, the pneumoconiosis patients' comprehensive assessment group formulated can evaluate the severity of pneumoconiosis patients, and make a more accurate and comprehensive judgement of the disease.

ABSTRACT

Objective To measure the reliability and validity of the Chinese version of the compre-hensive assessment of ACT processes(CompACT)in financial staffs. Methods A total of 3 735 valid ques-tionnaires were obtained from financial staffs.The valid questionnaires were randomly allocated into two groups,of which one subset(n=1 873)was used for exploratory factor analysis(EPA),and the other(n=1 845)for confirmatory factor analysis(CFA).Criterion and convergent validity were tested by Pearson corre-lation respectively.Incremental validity was tested by hierarchical regression analysis. Results The EFA suggested theoretically-coherent three-factor structure for a 15-itemed version of the CompACT.The three fac-tors named as acceptance and cognitive defusion,mindfulness and self as context,value and committed ac-tion,and explained 73.75% of the total variance and factor loadings ranged from 0.67 to 0.90.The CFA con-firmed the hypothesized three-factor mode(χ2/df=5.91,CFI=0.98,TLI=0.98,RMSEA=0.05). Conclu-sion The research suggests that the Chinese version of the CompACT has acceptable psychometric in prop-erties,so it can be applied in the assessment of the psychological flexibility and mental health in China.

ABSTRACT

Prostate cancer (PCa) is the primary malignancy threatening men's health in the United States and its incidence is increasing in China year by year. Many PCa cases are found in the advanced stage or in the old age, treated with unstandardized options, with negligence of comprehensive assessment of the patient's physical status, which may frequently add to the physical, psychological and economic burdens of the patient and even result in death. On the other hand, some excessively conservative therapeutic options may cause the loss of the best time of management or lead to deterioration of the disease. Therefore, sufficient importance should be attached to the comprehensive assessment of the physical status of the elderly PCa patient before treatment and the choice of the best therapeutic protocol.

ABSTRACT

Four foreign medical electronic journals databases were comprehensively assessed and analyzed using the assessment index system developed by the Library of Capital Medical University.After the raw data of 28 assessment indexes covered in the 4 databases were transformed to dimensionless data, the service performance of each database was scored with suggestions proposed for the future acquisition of digital literature resources.Our empirical study showed that assessment of database could contribute to understanding the quality of foreign medical electronic journals databases and how they could meet the need of users.

El artículo presenta la valoración de los servicios ecosistémicos de investigación y educación en la microcuenca de la quebrada Dalí, ubicada dentro de la hacienda Lisbrán; a través de tres objetivos específicos: primero, identificación de información biofísica y socioeconómica del área de estudio (línea base); segundo valoración integral del servicio de "investigación y educación" ofrecido por la microcuenca; por último analizar cómo los servicios ecosistémicos y la educación ambiental pueden convertirse en un insumo para la gestión local. Para el primer se identificaron y clasificaron algunos servicios ecosistémicos mediante información secundaria. Estos fueron evaluados mediante los métodos de ranking y rating, obteniendo los mayores puntajes los servicios de oferta y regulación hídrica, y como segundo y en el cual se fundamenta el artículo los servicios educación y de investigación. Para el segundo se realizó una valoración integral del servicio de "investigación-educación" en sus aspectos ecológicos, sociales y económicos, tomando como objeto de estudio siete programas (pregrado y posgrado) de la Universidad Tecnológica de Pereira. El tercer se desarrolló con la participación de los scout "rama de lobatos" como parte del diseño de un proyecto de educación ambiental que ellos y otras partes interesadas fomenta la importancia de los recursos naturales y los beneficios de la sub-cuenca Dalí como proveedor de servicios ecosistémicos. Los resultados muestran que el área de estudio tiene un gran potencial para ser usada como estación de investigación y sitio de prácticas para estudiantes de pregrado y posgrado. A pesar del hecho de que los profesores y estudiantes entrevistados reconocen la importancia de los atributos ecológicos que ofrece la subcuenca de la quebrada Dali, ninguno de ellos expreso disponibilidad a pagar por usar este sitio como lugar para desarrollar sus investigaciones o tesis de investigación. Sin embargo, la experiencia del trabajo con niños de los grupos scouts en el sitio de estudio sugiere que el sitio tiene un gran potencial no solo para hacer investigación sino también para para la educación ambiental


The article presents the valuation of the research and education ecosystem services in the Dali mountain stream, located in the Lisbran farm through three specific objectives: first, to identify the biophysical and socioeconomic information of the area of study (Baseline); second, to assess comprehensively the "research and education" service offered by the micro basin; and third, to analyze how the ecosystem services and environmental education can become input for local management. Some ecosystem services were identified and classified through secondary information to accomplish the first objective. These were evaluated using the ranking and rating methods, obtaining the highest scores on services of water supply and, in second place, research and education services in which this article is based. A comprehensive assessment of "the research and education" service considering its ecological, social and economic aspects was carried out to achieve the second objective. Seven undergraduate and graduate programs from Universidad Tecnológica de Pereira were taken as the object of study. The third objective was developed with the participation of the cub scouts group as part of the design of an environmental education project that, as well as other stakeholders, encourage the importance of natural resources and the benefits of the Dalí mountain stream as a supplier of valuable ecosystem services. The results show that the area of study has a great potential to be used as a research and educational station as well as internship site for undergraduate and graduate students. Despite the fact that the professors and students interviewed recognize the importance of the ecological attributes offered by the Dali mountain stream, none of them expressed the willingness to pay for the use of this site as a location to conduct their research and thesis projects. However, the experience of working with children from the scout groups suggests that the site has a great potential not only for research but also for environmental education activities.

ABSTRACT

To provide a scientific basis for the selection and optimization of the modern drying processing method for Angelicae Sinensis Radix (ASR). Three phenolic acids (esters), 6 phthalides were determined by using UPLC-PDA while polysaccharides were determined by UV-Vis spectrophotometry. Then the effects of drying methods on the inner qualities of ASR were evaluated through principle components analysis (PCA) combined with the appearance properties after drying. Results showed that the contents of chlorogenic acid and ferulic acid in samples obtained with controlled temperature and humidity drying (CTHD), medium and shortwave infrared drying (MSID) and microwave vacuum drying (MVD) methods were significantly higher than those with primary drying processing(PDP) method and the fresh samples. Multivariate statistical analysis showed that samples processed with CTHD had more similar general chemical properties with those processed with PDP, suggesting that CTHD was appropriate for the modern primary drying processing of ASR. With samples processed with traditional PDP method as reference, the CTHD method was further optimized in the processing parameters for ASR by orthogonal experiment design. Considering the consumption of drying power and time and other parameters, the modern drying parameters for the primary drying processing of ASR were finally optimized as follows: controlled temperature and humidity drying at 40-45 ℃, relative humidity below 25% and target moisture content about 50% in the first stage of drying process, tempering for 12-24 h, and then drying under the conditions of temperature at 50-60 ℃, relative humidity below 20% and fan frequency at 30-40 Hz in the second stage. The study provided the scientific evidence for the selection of appropriate drying method and suitable parameters for the modern primary drying processing of ASR, as well as the beneficial exploration and practice on the formation of technical standard of primary drying processing for roots and rhizomes types herbal medicines.

Objective To investigate the situation of admission nursing evaluation on elderly patients of geriatrics departments in several Shanghai comprehensive hospitals, and the results will be the base for designing elderly patient′s comprehensive admission nursing evaluation form. Methods 41 nurse managers of geriatrics department in Shanghai comprehensive hospitals were selected as research object. Self-designed questionnaire included general data, elderly patient admission nursing evaluation content and the importance of each item. Results The effective recovery rate was 97.56%(40/41). 98%(39/40) hospitals considered that elderly patients need to have a comprehensive assessment according to the elderly characteristics, only 18%(7/40) hospitals used the special nursing evaluation form for elderly patients. The score of importance of five dimensions were as follows: geriatric syndromes or problems scored (4.64 ± 0.38), physical function assessment scored (4.60 ± 0.46), physiological function assessment scored (4.53 ± 0.45), mental and psychological assessment scored (4.10 ± 0.57) and social assessment scored (4.07±0.52). The score of importance of 29 items were as follows:fall assessment scored (4.90±0.30), pressure ulcer assessment scored (4.85 ± 0.36), activities of daily living scored (4.75 ± 0.44), urinary incontinence assessment scored (4.70 ± 0.52), pain assessment scored (4.70 ± 0.46). Conclusions It is of great significance to carry on the comprehensive admission nursing evaluation for the elderly patients, which needs to be valued and strengthened.

Objective To analyze the change in qualification rate of comprehensive assessment , and to provide an effective theoretical basis for future selection and recruitment of military flying cadets .Methods Group A consisted of 400 cadets who were randomly chosen from the final selection with the physical examination conclusions of disqualifiedandqualified by comprehensive assessmentin a certain year via the cross-sectional investigation method , and group B was composed of 400 cadets who were chosen from the following year with the same method as group A .A questionnaire con-cerning demographic features was conducted among all cadets .A database was established with Epidata 3.02 software, and SPSS 16.0 software was used to analyze diseases which were qualified by comprehensive assessment.Results The qualification rate of comprehensive assessment in group A was higher than group B (45.00%vs 34.75%, P<0.01),there were differences between the two groups in overweight ( underweight ) ,deflection of nasal septum , gallbladder polyp , spinal malformation and undertall(P<0.05 or P<0.01).Conclusion The expert consensus which is made by the air force ex-perts through their work experience and the foreign air force standards has a positive application effect in the practical work . It is an efficient supplement to the physical examination standards and has an important effect on military aviator selection .

Objective To provide an effective theoretical basis for future selection and recruitment of military flying cadets.Methods Two hundred military qualified flying cadets by comprehensive assessment samples and 200 military flying cadets by medical fitness samples were interviewed with questionnaires and subjected to a physical examination.The comprehensive assessment samples were classified by symptoms and attributive specialties for statistical analysis.A database was established, cleaned and analyzed by EpiData 3.02, SPSS 16.0 with double checking.Results There was no differ-ence between the comprehensive assessment team and the medical fitness team in age, areas, academic scores and psycho-logical performance.The ten top symptoms according to the comprehensive assessment were abnormal chest circumference, abnormal body mass,iris cyst,abnormal hypersplenotrophy,gallbladder polyps,flat foot,skin disease, intrahepatic calcifica-tion focus, joint disease and reproductive system disease.The attributed specialties included surgery, ultrasonics, ophthal-mology,ENT,radiology, ECG and internal medicine departments.The surgery and ultrasonics department contributed the most from the comprehensive assessment and accounted for 55.52%and 16.37%, respectively.Conclusion The compre-hensive assessment for military flying cadets has an important effect on military aviator selection.Continuous observation should be carried out among the military flying cadets by comprehensive assessment in their aviation training.

Background Evaluation of patient outcomes following transcatheter aortic valve implantation (TAVI) has usually been based on sur-vival and clinical improvement. Studies on quality of life are limited, and data from comprehensive assessments after the procedure are lack-ing. Methods Sixty patients referred for cardiac rehabilitation after TAVI underwent in-hospital and after-discharge multidimensional as-sessments to evaluate clinical, functional, and nutritional statuses, degree of autonomy, cognitive impairment, depression and quality of life. Results On admission to rehabilitation, approximately half of the patients had severe functional impairment and dependence for basic ac-tivities of daily living. During their hospital stay, one-third of the patients suffered significant clinical complications and two had to be trans-ferred to the implantation center. Despite this, the overall outcome was very good. All of the remaining patients were clinically stable at dis-charge and functional status, autonomy and quality of life were improved in most. During a mean follow-up of 540 days (range:192–738 days), five patients died from noncardiac causes, three were hospitalized for cardiac events, and nine for non cardiac reasons. Functional status and autonomy remained satisfactory in the majority of patients and most continued to live independently. Conclusions Patients re-ferred for rehabilitation after TAVI are often very frail, with a high grade of functional impairment, dependence on others and high risk of clinical complications. During a rehabilitation programme, based on a multidimensional assessment and intervention, most patients showed significant improvement in functional status, quality of life, and autonomy, which remained stable in the majority of subjects during mid-term follow-up.

Pearson correlation analysis was used to students' formative assessment and summative as-sessment, and comprehensive evaluation achievement(the first two added). Moreover, the effects were assessed by teachers' self-evaluation and their exchanging ideas with students. Results Ranging from 10.06 to 12.22, the standard deviations of four grades' summative assessment results were the biggest number, so the summative assessment was more effective. Students' formative assessment, summative assessment, and comprehensive assessment for this course were positively correlated(P<0.01) for suc-cessive four years. Teachers and students approved of the new mode. Conclusion The effective com-bination of formative assessment and summative assessment was helpful for student to develop and assess learning ability and overall qualities, and for teachers to improve teaching level.

ABSTRACT

On May 3rd, 2003, a strong earthquake hit the eastern part of Turkey at 03:27 a.m. and lasted 20 seconds. It destroyed a regional boarding school killing 83 students and 1 teacher. Disaster-exposed children are at risk for a variety of mental health, social, and academic problems. This research was designed to investigate the extent of comorbidity among adolescent earthquake survivors. One hundred and ninety-one adolescents were given the Child Post Traumatic Stress Reaction Index and the Behavior Assessment Systems one year after the earthquake. The adolescents were divided into PTSD (Post Traumatic Stress Disorder) positive and non PTSD groups. Multivariate ANOVAs were then conducted to test group and gender differences on the BASC subscales. Correlation analysis revealed some significant associations between the CPTSD RI and BASC subscales. Multiple stepwise regression analysis was used to predict the contribution of each of the BASC SRP A subscales. Depression was the strongest contributor, accounting for 23% of the total variance. The next most significant contributing variable was atypicality, at 6%. This was followed by sensation-seeking, with a 4% variance. Finally attitude to school added another 2% to the prediction for PTSD. These four variables, together, explained 35% of the variance in the CPTSD RI total score (r.585, r²=.35 p<.05). Those variables also correlated with the CPTSD RI subscales of re-experiencing, avoidance and hyperarousal. Cross- cultural implications were also discussed in reference to the disaster and other disorders.


El 3 de mayo de 2003 un fuerte terremoto de 20 segundos golpeó Turquía del este a las 3:27 de la madrugada, éste destruyó un internado regional en donde 83 estudiantes y 1 maestro fallecieron. Los niños expuestos a desastres están en riesgo sufrir una variedad de dificultades con respecto a su salud mental, así como problemas sociales y académicos. El propósito de este estudio fue evaluar el grado de comorbilidad entre los adolescentes sobrevivientes al terremoto. Un año después de la catástrofe 191 adolescentes completaron el Índice de Reacción de Desorden de Estrés Postraumático en Niños CPTSD y el Sistema de Evaluación de Conducta BASC. Estos adolescentes fueron divididos en grupos de PTSD (Desorden de Estrés Postraumático) positivo y grupos que no presentaban PTSD. De esta manera, se llevaron a cabo análisis de varianza ANOVA para probar las diferencias de grupo y de género en las subescalas BASC (Sistema de Evaluación de Conducta para Niños). El análisis de correlación reveló algunas asociaciones significativas entre el CPTSD RI y las subescalas BASC. Múltiples análisis de regresión stepwise (paso a paso) fueron utilizados para predecir la contribución de cada una de las subescalas BASC SRP A. La depresión fue el mayor contribuyente, lo que representa el 23% de la varianza total. La siguiente variable que contribuye significativamente fue atípica al 6%. Esto fue seguido por la búsqueda de sensaciones, con una variación del 4%. Por último, la actitud hacia la escuela añadió un 2% a la predicción para el PTSD. Estas cuatro variables juntas explicaron el 35% de la varianza de la puntuación total del CPTSD RI (r.585, r²=.35 p<.05). Estas variables también se correlacionan con las subescalas de RI CPTSD de re-experimentación, evitación e hiperexcitación. Las implicaciones transculturales también fueron discutidas en referencia a la catástrofe y otros trastornos.

ABSTRACT

OBJECTIVES: The aim of this study is to investigate the proportion of dementia studies and the trends of the use of neuropsychological tests in dementia studies for the past 10 years. METHODS: The subjects of this study were 715 original articles published in Journal of Korean Neuropsychiatric Association and 72 original articles published in Journal of Korean Geriatric Psychiatry between January 1, 2000 and December 31, 2009. Among those, there were 78 articles of dementia study and 36 articles using the neuropsychological test used for the analysis. According to the published year, all articles were divided into two stages (between 2000-2004, between 2005-2009). The data were collected for what neuropsychological tests were used. RESULTS: The proportion of dementia studies increased from 8.2% to 12.4% and the proportion of dementia studies using the neuropsychological test decreased from 60.5% to 37.5%, when comparing the two stages. The use rate of screening tests, intelligence tests, and domain-specific cognitive tests decreased from 91.3% to 84.6%, 13.0% to 0%, 91.3% to 76.9% respectively, whereas the use rate of comprehensive assessment of dementia increased from 21.7% to 38.5%. CONCLUSION: These results show an increasing trend in dementia studies and decreasing rate of neuropsychological test use in Korea. However, the use rate of comprehensive assessment of dementia increased.
